Glenn D. Warden, MD, MBA - Chairman
Glenn Warden received his M.D. from the University of Utah in 1968.
His distinguished career in burn surgery began with a research fellowship
at the United States Army Institute of Surgical Research Burn Unit
at Brooke Army Medical Center (1971-1974). In 1976, he was appointed
to the surgical faculty at the University of Utah, rising to full Professorship
in 1985. He served as Director of the Intermountain Burn Center and
the Trauma Division in the Department of Surgery from 1977 to 1985.
In 1985, he became Professor of Surgery at the University of Cincinnati,
College of Medicine, Director of the Burn Division of the Department
of Surgery and the Chief of Staff of the Shriners Hospital for Children
- Cincinnati Burns Hospital. He was appointed Vice-Chairman of the
Department of Surgery in 1997. In 1999, he received a Masters of Business
Administration from Xavier University in Cincinnati, Ohio. Dr. Warden
has authored over 600 manuscripts in burn care and serves as Emeritus
Editor-in-Chief for the Journal of Burn Care & Rehabilitation.
He was President of the American Burn Association (ABA) from 1992-1993
and recipient of the coveted Harvey Stuart Allen Distinguished Service
Award in 1997 and the American Burn Association Special Achievement
Award in 2006. He chaired the General and Plastic Surgery Devices Council
of the Food and Drug Administration in 1992 and contributes to several
medical advisory boards and 39 professional societies. Dr. Warden is
currently Adjunct Professor of Surgery, University of Utah, Professor
of Surgery Emeritus, University of Cincinnati, Emeritus Chief of Staff
of the Shriners Hospital for Children, Cincinnati Burns Hospital and
Staff Surgeon, Intermountain Shrine Hospital for Children, Salt Lake
City, Utah.
Richard Anderson, MD, AOA - Scientific Advisor
Dr. Richard Anderson graduated AOA from the University of Iowa Medical
School. He completed surgical internship and ophthalmology
residency at the University of Iowa. He completed fellowships in ophthalmic
plastic and reconstructive surgery at Albany Medical Center and the
University of California San Francisco. He returned to the University
of Iowa in July 1976 to begin his academic career as Assistant Professor
and Director of the Oculoplastic and Orbital Service at the University
of Iowa. He was a prolific writer and innovator in his field
and became full professor and chief of the Division in July 1982. In
1984 he moved to Salt Lake City to begin private practice and was appointed
Professor and Chief of the Division of Ophthalmic Plastic and Facial
Cosmetic surgery at the University of Utah, which position he maintained
until 1999. In 1999 he became medical director of Oculoplastic Surgery,
Inc. and The Center for Facial Appearances in Salt Lake City, UT which
position he maintains to date. He is board certified and a Fellow of
the American Academy of Ophthalmology, the American Academy of Facial
Plastic and Reconstructive Surgery, the American Society of Ophthalmic
Plastic and Reconstructive Surgery and the American College of Surgeons.
At the University of Iowa and then at the University of Utah, Dr. Anderson developed oculoplastic divisions with one of the largest volume in the country. He has trained over fifty U.S. and foreign fellows, many of whom direct oculoplastic programs around the country and world. He has published over 300 scientific papers, book chapters and 2 books in oculoplastic, facial plastic, and cosmetic surgery. Dr. Anderson has developed many modern day techniques in oculoplastic, facial plastic, and cosmetic surgery. He has delivered over 700 papers at scientific (national and international) meetings. He has been or is presently on the editorial board of 11 journals in the field, including Archives of Ophthalmology, Ophthalmic Plastic and Reconstructive Surgery and Archives of Facial Plastic Surgery.
In the past 15 years Dr. Anderson has been named to the top of his field by a number of organizations. In 1991 Good Housekeeping Magazine and Reader's Digest Magazine named him to the best 400 Doctors in America, as the only Oculoplastic Surgeon named to that group. He has been named to the Best Doctors in America yearly. In 1998 Dr. Anderson was honored by Ophthalmology Times as the top doctor in oculoplastic surgery. He received the Achievement in Ophthalmology Award as 1 of 5 doctors to make the greatest contributions in their fields. He has received many other honors and awards within his field including The Heed Foundation Award for outstanding contributions to ophthalmology, The American Academy of Ophthalmology Honor Award and Senior Honor Award and The Lester Jones and Wendell Hughes Awards. Dr. Anderson has been a member of many medical advisory boards including the Benign Essential Blepharospasm Research Foundation and is on the board of directors of the aforementioned foundation. He is a past vice president of the American Society of Ophthalmic Plastic and Reconstructive Surgery and a member of the prestigious Orbital Society.
Kevin P. Yakuboff, MD, ACS - Scientific Advisor
Kevin
P. Yakuboff MD received his MD degree from St. Louis University School
of Medicine in 1977 and completed a General Surgery Residency at the
University of Cincinnati in 1982. He served as Chief of Surgery at
Grand Forks Air force Base Hospital (SAC) from 1982-1986. He returned
to the University of Cincinnati and completed a residency in Plastic
Surgery in 1988. Following residency he completed postgraduate fellowships
in aesthetic surgery, Microsurgery and the Christine Kleinert Fellowship
in Hand and Microsurgery in Louisville, Kentucky.
In 1988, he returned to the Cincinnati where is now Associate
Professor Surgery and Director of Hand and Microsurgery at
the University of Cincinnati Medical Center, director of Brachial Plexus
Injury program at the Children's Hospital Medical Center and Chief
of Plastic surgery at the Shriners Hospital for Children, Cincinnati
Burns Hospital.
His academic interests include burn reconstruction, hand surgery, microsurgical reconstruction of pediatric burn problems, peripheral nerve surgery and brachial plexus reconstruction. He is active in the National Organizations and has numerous abstracts, publications and book chapters. He serves numerous national committees including the American society of Reconstructive Microsurgery, ASPRS Hand Panel, and the Education committee of the American Burn Association. He is also on the Editorial Board of the Journal of Plastic and Reconstructive Surgery and the Journal of Burn Care and Rehabilitation. He has volunteered numerous times as a member of the Interplast Cleft Lip & Palate Team to San Fernando, Philippines.
Mark B. Taylor, M.D - Scientific Advisor
Mark
B. Taylor, M.D. is a world-renowned dermatologist and cosmetic
laser surgeon who has been in practice for over thirty years. Since
the origination of laser treatments for skin, Dr. Taylor has been a
leader in the field of cosmetic laser surgery, pioneering and teaching
many new laser techniques to over 4,000 doctors world-wide.
Dr. Taylor received his medical degree from the University of Utah and is board-certified in dermatology through the American Board of Dermatology. He is a member of the American Society for Laser Medicine and Surgery as well as many other professional organizations. He was the first doctor in the world to use the Krypton and VeinLase lasers for the treatment of vascular and pigmented lesions. Dr. Taylor was one of the first researchers to use the PhotoDerm, Derma K, Cynergy and MX 1540 Lasers. Many physicians have traveled to Gateway for instruction from Dr. Taylor on advanced techniques in laser resurfacing. In addition, Dr. Taylor has developed new techniques for many other laser treatments and cosmetic surgery which he performs on a regular basis in his busy practice in Salt Lake City.
James W. Roach, MD - Scientific Advisor
James
W. Roach MD received his MD degree from Georgetown University
in 1974 and completed an Orthopedic Surgery Residency at the Tripler
Army Medical Center, Honolulu, Hawaii in 1978 followed by a fellowship
in Pediatric Orthopedics at the Texas Scottish Rite Hospital, Dallas,
Texas.
He remained on the staff at the Texas Scottish Rite hospital until 1993 and when he became Director of Orthopedic Surgery at the Cook children's Medical Center in Fort Worth, Texas until 2001 when he became Chief of Staff at the Shriners Hospital for Children, Intermountain Hospital in Salt Lake City, Utah. He serves in that position at the present time and is also Professor of orthopedic Surgery at the University of Utah School of medicine.
His academic interests include Scoliosis, congenital hip dislocation and postoperative pain management in children. He is active in national organizations including the American Academy of Orthopedic Surgeons, American Orthopedic Association, Pediatric Orthopedic Society of North America and Scoliosis Research Society and has served on numerous committees in these organizations. Dr. Roach has numerous abstracts, publications and book chapters. He has served as a visiting surgeon in Georgetown, Guyana, Chonju, Korea and recently in Iraq.
R. Scott Ward, PhD - Scientific Advisor

Scott Ward PhD received his BA in Physical Therapy at the University
of Utah in 1980. Following graduation he became Associate Director,
Burn Therapy Department, Intermountain Burn Center, University of Utah
Medical Center from 1981 to 1984. During this time he was a Clinical
Instructor, Division of Physical Therapy, University of Utah and Weber
State College, Ogden Utah. His interest in the pathophysiology of the
burn patient prompted Dr. Ward to pursue a PhD in physiology which
he received in 1994. He was Co-Chair, Division of Physical Therapy,
College of Health, University of Utah from 1990 until in 1997 when
he became Chairman of the Division of Physical Therapy, College of
Health, University of Utah. In 2004 he was awarded Professor, Division
of Physical Therapy, College of Health, University of Utah. He has
been active in various national organizations. He was President of
the American Physical therapy Association in 2006 and a member of the
Board of Directors of the American Physical Therapy Association from
2001 – 2006. He has been very active in American Burn Association.
He was a member of the Board of Trustees of the American Burn Association
from 1994- 1997. He also served on the Program Committee and the Board
of the American Burn Association Education and Research Foundation.
He is currently the Chairman of the Rehabilitation Committee. He has
numerous publications including 14 book chapters, 33 publications and
36 abstracts. He received the Lucy Blair Service Award, American Physical
Therapy Association in 1999 and the Barbara Knothe Burn Therapist Achievement
Award from the American Burn Association in 2003.
